Community Living Program
The Community Living Program supports adults with cognitive disabilities through a variety of services that promote independent living. We assist our clients in housing searches for safe and affordable housing, and support them in managing their finances, housekeeping, personal hygiene, developing leisure-time activities, securing health services, and other daily living skills training that they may need.
We also offer the only program in Milwaukee County that supports families headed by parents with cognitive disabilities. Our staff members assist and support the parents with parenting skills, advocacy in their children’s education, support with the Bureau of Milwaukee County Child Welfare System, and daily living skills for which they may need training.

Family Care Program
The Family Care Program provides case management services to individuals over 60 years of age, who live in the community and receive services funded through the Milwaukee County Department on Aging (MCDA) Family Care - Care Management Organization (CMO). Most members served in this program have a primary diagnosis of a developmental disability and/or mental health disability, although the frail elderly population is served as well.
The primary goal of the Family Care Program is to ensure that members receive necessary and appropriate services by honoring and promoting member's continued participation in the life of their community and by providing a continuum of quality, cost-effective, long-term care options. Care managers ensure that all members maintain eligibility for Family Care services, both financially and functionally, and they work carefully with service providers and the CMO Administration to ensure that members remain living safely in the least restrictive environment.

Long Term Support Service Coordination
The Long Term Support Service Coordination Program provides case management services to individuals receiving CIP waiver-funded services through the Milwaukee County Disability Services Division. The program assists adults with developmental disabilities to remain in the least restrictive living environment, promotes independence and coordinates residential placements, work and day program activities, and recreation services.
To participate in the Long Term Support Program, you must be referred through the Milwaukee County Disability Services Division and be receiving CIP waiver services.
