MCFI Comprehensive Care Connection is your in-home link to health and wellness planning solutions. Our professional staff has the expertise to work with older adults and people with a variety of special needs to help maintain quality independent living.
MCFI Comprehensive Care Connection is committed to providing expert assessment of needs and coordination of services for older adults and people with special needs and their families. Our goal is to help you remain independent in your home.
We will help you develop a comprehensive plan to identify health, wellness and living care needs, both now and in the future. Experienced staff members will work with you to create a customized care plan, including providing service, coordinating services from other sources and providing monitoring and oversight of all service providers.
For family members who need assistance caring for an adult or older adult from a distance, MCFI Comprehensive Care Connection can be your onsite coordinator and your liaison to doctors, attorneys, insurance companies and other service providers needed to ensure quality care.
Health and medical management
- Assist adults and older adults and their families using an interdisciplinary team of health and social service professionals
- Conduct assessments to identify problems and provide solutions
- Screen, arrange and monitor in-home help or other services (including medication management, shopping assistance, transportation to medical and other appointments, meal preparation, personal care, homemaking companion care, respite care, seasonal outdoor assistance, pet sitting)
- Provide education and consultation to assist individuals and families to help themselves
- Offer counseling and support to help individuals cope with changes and families to work together
- Specialized services for people with disabilities
- Specialized services for individuals with dementia/memory loss
- Provide services as needed, 24 hours a day/seven days a week, including crisis intervention assistance
Distance Care
- Act as a liaison to families at a distance, overseeing care and alerting families to problems
- Assist with moving/relocating (including to or from a retirement complex, assisted care home or nursing home)
- Arrange and oversee the health and medical management services previously listed
Medical, legal and financial assistance
- Review medical, legal or financial issues and offer referrals to specialists as needed
- Arrange transportation to appointments (medical, dental, legal, insurance, etc.) and assist with communication of information as needed
